    • This is the ONLY Physical form that will be accepted for participation next year. The only form acceptable is the one that has “Revised April 2007” in a box on the upper left hand corner of the first page. It also has a VHSL emblem on the upper right hand corner of the first page.

    • Information regarding sponsorship of Hawks Athletics.
  • Game Summary Sheet - (MS Word )
    • To be filled out by coaches and returned to the Activities Office by 9:00 am the next morning.
  • 2010-2011 Extra Curricular Participation Policy - (PDF )
